Welcome to the official website for Justin and Michael's babysitting service! We hope
you enjoy your visit. If you would like us to babysit for you, you can also complete the online application form. Before we
babysit, we ask that you complete the Babysitter's information sheet so that we know how you would like us to handle medications,
discipline, and other things we may need to take care of during our job. Justin has lots of experience with first aid and
emergency preparedness from his Boy Scout activities in the troop. We hope you consider Justin and Michael's babysitting service

Attention: We require that we have a document signed by the parent authorizing us to take the neccesaary steps to administer
first aid, seek medical assistance, and give the child any medication specified by you. For safety reasons, we
cannot give any medications that are not specified on the sheet, and we cannot begin babysitting without the signed form! Although
we only require for the Family Information Form to be completed, we highly reccomend completing the Parental Consent
and Contact form, Family Interview form, and Safety Inspection Checklist before we begin babysitting. This
